Contact and Declaration form

Keeping records up to date assists us in contacting the right people within your organisation. This year’s contact form was made available earlier in the year and it would be appreciated if a completed form could be uploaded via the Employer Portal if you haven’t already done so. All sections of the form must be completed with the relevant contact details in full, so please don’t enter “no change”. Please note that updates can be made throughout the year as changes occur.
